Request a fire report

You can request a medical emergency, fire incident, and fire investigation report by filling out the Report Request form and emailing it to firereports@hayward-ca.gov, or fax to 510-583-3641.  

You can request fire or a hazardous materials file review by calling our Fire Prevention Office at 510-583-4900, email haywardfirepermits@hayward-ca.gov. Or you can come into the office at:

Monday - Thursdays from 8 a.m.–1 p.m.
